Please help me with my dissertation on surfing injuries.

Hi

I am a keen surfer from North Devon and am currently doing a Foundation Degree in Sport and Exercise Rehabilitation.

For my dissertation I have to find out about surf related injuries and the causes behind them.

I know I am new to the forum but would greatly appreciate anyone who has been injured to fill out the questions below.
One of the stipulations of my course is that the questionnaires have to be confidential, so please could you copy and paste

the questionnaire into a PM for me?

It looks long, but I promise it doesn't take more than a couple of minutes
Thanks loads in advance.

Hayley
